Form examples, one for each thing a small business site takes in

Most form examples on the web are screenshots of a builder's template gallery, and they answer the wrong question: they show what a form can look like, not what it needs to ask. The forms samples on this page are the six forms a small business site runs, each given as its list of fields, which fields are required, what the confirmation says and the one figure that decides whether it is working. They are the contact, the quote request, the payment, the order, the RSVP and the intake, and each is worked on the free sheet on this site that sizes it, so the example is a number as well as a list.

The contact form and the quote request form

The contact form is name, email, phone optional, and a message: four fields, two required, 36 seconds on the contact form design sheet, and on a page with 1,200 visitors and the worked example's rates it produces 36 entries a month. The quote request form is the contact form plus a service dropdown and that service's three questions, shown by conditional logic: 17 fields built, 8 seen, 72 seconds on the request a quote form sheet, and 30 requests a month at a $650 job is $19,500 quoted.

The payment form and the order form

The payment form is a line or two with quantities, an option, and the card: on the payment form calculator two of a $45 line, one of a $120 line and a $15 option is a $225 subtotal, $243.56 charged at 8.25% sales tax, $7.36 to the processor at 2.9% plus $0.30 and $236.20 reaching the account. The order form is the same form with the menu as the lines and a pickup time added, and the same arithmetic decides what lands.

The RSVP form and the intake form

The RSVP is a name, a yes or no and a count, with the meal and the extra guests behind the yes: three fields on the first screen. The intake form is the longest of the six and the one that most needs steps: the contact details, then the matter, then the documents, with the fields per step kept to what fits a phone. Every entry from every one of these six forms is kept with its status in Shortcodo Pro, and the sheet each form was sized on is kept beside it.

Questions people ask about form examples

Which of the form examples should a new site start with?

The contact form, sized on the contact form design sheet. Every other form is that form with a job added.

Are these forms samples for WordPress only?

The fields are the same in any builder. The shortcode is how a WordPress page places the form; the arithmetic does not care.

Where do the figures in the examples come from?

From the three sheets' worked examples, and every one changes when you enter your own prices, rates, fields and traffic.
