Three sheets, one for the money, one for the enquiry and one for the quote. The payment form calculator is the charge: the lines times their quantities plus the option fee is the subtotal, the subtotal times your sales tax rate is the tax, and the two together are what the form charges unless you pass the processor's fee on, in which case the charge is grossed up so the subtotal and the tax still reach you. The contact form design sheet is the enquiry: fields times seconds is the time to fill, visitors times the start rate is the starts, starts times the finish rate is the entries, and the difference is what abandonment costs you at your own enquiry value. The request a quote form sheet is the quote: the fields every quote needs plus the questions for one service is what a visitor sees when conditional logic hides the rest, and the requests a month times your win rate and job value is the work the form wins.

These sheets have no constants. Every number is worked from the figures you enter: the prices, the quantities, the option fee, your sales tax rate, your processor's rate and fixed fee, the fields, the seconds per field, the visitors, the start and finish rates, the services, the questions, the requests, the win rate and the job value, with the arithmetic printed beside each answer. The defaults are one site owner's worked example, not a tax rate this site publishes and not a processor's price list.
