These sheets have no constants. Every number is worked from the figures you enter: the prices, the quantities, the option fee, your sales tax rate, your processor's rate and fixed fee, the fields, the seconds per field, the visitors, the start and finish rates, the services, the questions, the requests, the win rate and the job value, with the arithmetic printed beside each answer. Conditional logic forms show a question only when an earlier answer makes it relevant, and for a small business site that is the difference between one form and five. A quote form with four services carries every service's questions, but the person quoting a fence sees the fence questions and nothing else; an event form asks for a second guest's name only when the party size is two; a payment form shows the delivery fee line only when delivery is picked. Rule one: show a field when an answer needs it
The service picked shows that service's questions; the party size shows the extra guest fields; the delivery choice shows the address. Each rule is one condition on one earlier field, and the field it reveals sits directly below the field that revealed it so the change is seen.
Rule two: require a field only when it is shown
A hidden required field is a form that can never be sent, and it is the commonest way a conditional form breaks. Every required mark on a conditional field is conditional on the same answer, and on the sheet the required share is printed against the seconds to fill so both are read together.
Rule three: route the entry by the answer
The service picked decides who is notified and what the confirmation says. On the worked example a contact form with 6 fields at 9 seconds each takes 54 seconds; hide two fields behind a condition and the form most visitors see takes 36, which is the figure a shorter form is measured against.
